Alternate Assessment Notification (PDF) / Alternate Assessment Notification Spanish (PDF) (January 2025)
The notification form should be provided to families during the IEP when a decision is made that a student needs to take an alternate assessment. This notification covers the requirements for criteria 5 on the DLM participation guidelines.
