Special Education Notices and Forms

Documentation for special education services is critical. The Parents Rights and Prior Written Notice are required to be provided in the native language of the parent. All the translations that KSDE has available are posted on the website. If the languages needed are not represented, the Kansas Association of School Boards, a large district, a cooperative or interlocal might be a good resource.

Procedural Safeguards (Parent Rights)

The Notice of Procedural Safeguards must be provided in the native language or other mode of communication used by the parent. 34 C.F.R. 300.504(d). KSDE has provided some of the languages spoken in Kansas. Any languages not listed here are not currently available from KSDE.

Notice of Meeting and Prior Written Notice

The Prior Written Notice must be provided in the native language or other mode of communication used by the parent. 34 C.F.R. 300.503(c)(ii). KSDE has provided some of the languages spoken in Kansas. Any languages not listed here are not currently available from KSDE. These are model forms only; LEAs are not required to use these exact forms.

Revocation of Consent - Particular Services

Revocation of Consent forms may be useful to parents and LEAs when a parent wishes to exercise the right to revoke consent for particular special education services and placements.
